Intake forms are available as a PDF on this page just below, which you can print and fill out prior to your appointment. Otherwise, it is also available in paper form in the clinic's front entry.

Reception: Although you will see a reception desk and someone working there, I do not use reception services. This is primarily for the naturopathic doctors. You're welcome to go say hi, but they don't know much about my business. I'll do all the check-in and check-out. 

Sessions: Sessions can range from 30 minutes to 2 hours. Occasionally the talking part is longer if there is a lot to discuss about your condition or goals. This may shorten hands-on time, but it is meant improve the overall quality of care.

Clothing: Please wear comfortable, loose-fitting clothing that can move with you and handle oil and herb stain. No jeans, skirts or excessive pockets. I also have clothing available for use if needed.

New patients can print off and fill out the following forms:

*NEW COVID FORM! Each person is required to fill this out for each office visit. 

For your first visit:

1. Intake Form-- all new patients and existing patients whose information has changed. 

2. Insurance Page- if we are billing your insurance. 

3. HIPAA-- No need to print this, it's just for you to be informed. 



Prices and co-insurance/co-pays vary based on length of treatment, insurance carrier and benefit package. Contact me for details. 

My current full billing price is $110/ hour (or 4 units). If you are paying in full at the time of service, you receive a discount off the full billing price: $80/hour. Any updated prices will be posted at the office prior to taking effect. 

Massage students

Get a one-time discount on their first 90 minute or 2 hour treatment when receiving as a class requirement.

Gift Certificates 

Are now available online through Square.

You can also purchase them in the office from me directly.  

Appointment Hours


Tuesdays, Wednesdays, Thursdays and Fridays

The earliest start time is 9:30 and latest end time is 7pm. 

Saturdays twice monthly

10:00am - 3:00pm

Portland location will no longer be available after July, 2019

Scheduling Tips

  • 24 hour notice is required for cancellations or changes. We reserve the right to charge for late cancel or missed appointments. Insurance companies do not reimburse for this: you will be billed directly. 

  • Please wear/bring comfortable clothing that allows for movement and is ok to stain. Some herbal products/oils may be used (with your permission only). Old or dark clothes, t-shirts, pajama pants, and the like usually work fine. 

  • Make sure you have time after your session for whatever you might need. When we stir up the physical stuff, a wide range of responses are possible, from soreness as your body readjusts to energy boosts to unblocked memories or emotions and more. 

  • If you are receiving massage for injuries sustained in a motor vehicle collision (car accident), please bring  a) a prescription for massage, and  b) your insurance and claim information including adjuster's name and contact information. More info on Insurance Billing page.

  • At my Gresham location, I can bill health insurance for qualifying plans. Or if I'm not contracted with your plan that has massage benefits, I am happy to provide you with a detailed receipt for you to submit for reimbursement. In some cases, this may save you money.
